FREE STUDY DAY ON PALLIATIVE CARE IN A CARE HOME SETTING

This is an invite to sign up for a free study day funded by NHS Education for Scotland in collaboration with Strathcarron Hospice and Scottish Care.

It is part of a range of opportunities being created to promote the capacity of care homes to deliver good quality palliative and end of life care, consistent with key Scottish Government policies.  These are being developed by the Palliative Care in Care Homes Steering Group, a partnership of Scottish Care, care home providers, NHS Education for Scotland, Scottish Government, the Care Commission, the Scottish Social Services Council and Cosla.

What is the day about?
The day will look specifically at anticipatory/advance care planning (ACP) which is an essential component of palliative care for those with life limiting conditions

ACP has been defined as a dynamic process of discussions between an individual, those who provide care and often those close to them about the person’s current and future care and wishes. It is a summary of ‘thinking ahead’ discussions and decision making that includes end of life care. An ACP is a reflective process and one that is voluntary, documentable and regularly reviewed.

ACP promotes:
•           A person-centred approach to care
•           Autonomy, dignity and opportunity for the individual to make positive choices (even if the person has lost decision making capacity)
•           Adding quality to living and dying
•           Effective communication between the individual, family and those who provide care.

Who should attend?
These study days are specifically aimed at colleagues in the care home setting who are currently involved in a leadership, management or training role within their organisation or care home, and who are able to influence and drive the future implementation of ACP within their care home setting.

As there will be a high demand to attend this study day, organisations are asked to nominate no more than one person per care home.

Where and When?
The study day will be held on 3 occasions:
