Updated Health and Social Care Data for Partnerships

Scottish Government has released updated Health and Social Care data for each Partnership in Scotland. It has been derived from publicly available national data sources and its primary purpose is to inform and advance the thinking of Partnerships regarding their local Change Plans.

The spreadsheet contains data on:

-Population projections
-Emergency admissions
-Home care services
-Care homes
-Free personal and nursing care
-QOF prevalence
-Delayed discharges
-Telecare services
-Respite care
-Percentage of lat 6 months of life spent at home or in a community setting
